Before you request a refill, have you checked with your pharmacy to see if you have any remaining refills on file? Before you request additional refills from your provider, be sure you have used all the refills sent to the pharmacy on your behalf at your last appointment. Only controlled medications have to be requested for refill monthly (ADHD, etc.).
Please complete this form only one time, as multiple requests will slow down the refill process. Medication requests should be submitted seven (7) days prior to running out of medication. Please do not wait until the last minute to request a refill, as providers are unable to accommodate last minute refill requests.
Please note that you must be seen by your prescriber on schedule in order to continue to receive refills. If you missed an appointment or have not come in for a follow up appointment according to the provider’s recommendations, refill requests will not be honored.
